Text Field Inputs:
Your Name: So we'll know who asked the question.
Your Street Address: This can give us some hints in dealing with technical matters. Mostly, it is used for people asking about cable modem access in their area.
Your Phone Number: If it is preferable for you to receive a response by telephone, we'll need the number to call.
Your E-mail Address: If you would rather a response over e-mail, or if you are reporting a problem with sending or receiving e-mail, this information will be required.
Detailed Description: Here's where you get to ask your question. Be as detailed as you possibly can. The more information you send us, the better the chance we have of being able to answer the question quickly and accurately.

Your Computer provides us with some information about your machine at home. Note: if you choose 'Other' as the type of system you have, please give details in the Detailed Description section.

Your Location tells us which of our dialup areas you are calling from. This helps us in troubleshooting modem problems in particular.

The third panel, Nature of Inquiry probably requires the most clarification. Shown are what we feel are issues dealt with by each category.
